If it wasn't documented, it didn't happen!  For years I carried around a small notepad in my shirt pocket.  If something happened or somebody said something I jotted it down right then, who said what, when and why.  Later it was transferred to my daily log or reports, but I never threw away that notepad.  When full, I would throw it in a drawer or box and later to an envelope.  You would be surprised how many times that note pulled my buns out of the fire!  I still carry a pocket pad.  In… Continue
